Report the Injury

  • Employee reports the incident immediately to his/her supervisor or department head, but no later than 24 hours after the incident occurs. Even if medical attention is not required, the incident must still be reported by the supervisor or department head as a report only.
  • The supervisor or department head completes and submits injury reporting forms to Human Resources:
    • “Employee’s Statement of Injury Facts” – injured employee completes with his/her supervisor or department head.
    • “Supervisor’s Report of Accident – to be completed by the supervisor or department head.

Seek Medical Treatment

  • If the injury is not urgent but requires medical attention, the employee will seek treatment at the City’s Designated Medical Provider (DMP) Amesbury Occupational Health Center located at 24 Morrill Place, Amesbury, MA. (978-834-8190). Open 8:00am to 4:30pm Monday – Wednesday and 7:00am to 3:30pm Thursday –Friday. It would be helpful for a representative from the employee’s department to contact the DPM prior to sending the employee for treatment.
    • For treatment of injuries when Amesbury Occupational Health Center is closed, the employee should go to the nearest emergency room.
    • If an employee does not seek treatment for a non-urgent injury from the City’s DMP, it may impact his/her workers’ compensation benefit coverage.
    • Referrals to specialists, physical therapy, etc. are made by the DMP treating physician.
  • If an injury occurs when the DMP is closed after the listed hours above, the employee should go to the nearest emergency room.
  • If an injury is severe and requires immediate attention, the employee should go to the nearest emergency room for immediate treatment.
    • A follow up with the City’s DMP, Amesbury Occupational Health Center must take place.
    • If the employee does not follow up with the City’s DMP, Amesbury Occupational Health Center, it may impact workers’ compensation benefit coverage.
  • If the treatment/diagnosis requires the employee to be out of work, it will be indicated on the paperwork issued from the City’s DMP, Amesbury Occupational Health Center. The treating physician will determine work status and required course of treatment, if any.
  • Note that the first five days of an injury must be covered by the employee's accrued sick time. The day of injury is considered a regular work day and the first full day after the incident begins the sick leave.

Payment

  • An employee should NOT use his/her health insurance card or pay any co-pays when seeking medical treatment related to the work related injury.
  • An employee should NOT pay any medical bills or invoices directly. All medical bills or invoices received should be forwarded to the Human Resources Department for payment: Human Resources Department, City Hall, 60 Pleasant Street, PO Box 550, Newburyport, MA. 01950, Attn: Chris Accardi

Communicate

  • If an employee is out of work, he/she must notify their supervisor and Human Resources with an update of their medical status as soon as possible. It is the employee’s responsibility to keep his/her supervisor and Human Resources informed of upcoming doctor appointments and medical status.
