COVID-19 Vaccine Records

Need a copy of your COVID-19 vaccination record? Here are three ways to get them:

• Access your Massachusetts digital vaccine card by clicking here.
• Request a copy of your record from the Massachusetts Department of Public Health by clicking here.
• Check with your healthcare provider. Most providers have access to the database that holds this information and will be able to pull your records and print them out for you.

PLEASE NOTE: The Newburyport Health Department does not issue replacement cards or retrieve records
